Request Records
If you are seeking transcripts and records for a current Shepherd Community Academy student who is transferring enrollment to your school, please submit a student records request to our Academy Director through the process below.
You can request records by:
- Sending an email to Academy@shepherdcommunity.org with subject line “Records Request: STUDENT LAST NAME, FIRST NAME”. Please also provide in your email a contact person, phone number, and preferred method of receiving records.
- Submit request by fax to (317)375-0205 Attn: Academy Director.

Please fill out 1 form per student requesting records. (i.e., For siblings, you must submit separate requests for each student.)
